Please fill out this form if an issue arises that needs our attention. Be mindful when selecting an urgency level. These levels have been established to ensure that problems reported receive adequate and timely attention based on business impact.

Level 1

Impact is minimal to your business and your members.  We will add this issue to your priorities to be scheduled.

Level 2 

Your business and/or your member experience is impacted but services can still be accessed. We will set this as one of your current priorities to be worked on within the next 5 business days.

Level 3 

Your business and/or your member experience is negatively impacted and access to service is limited. We will set this as a high priority item to be worked on within the next 2 business days.

Code Red

A critical site function is failing. Your business and/or your members cannot access services. Issue is of the highest priority and will be worked on immediately.
